What are the most pressing health needs in our community? Let Northern Inyo Healthcare District (NIHD)  know by taking its Community Health Needs Assessment (CHNA) survey. The survey will
run from May 2 to June 3.

The 10-minute anonymous survey asks questions about what you think are the most important health issues in our community and what matters to you most.

The survey is available in English and Spanish. Paper versions of the survey are available at all
admission desks within NIHD.

The CHNA survey results will assist with the development of NIHD’s community benefits plan,
involving many community partners, to improve the health of our community. The survey is also important because its results can affect funding, spending, and other wide-reaching decisions about health care.

Please share the survey with your colleagues, friends and family and encourage them to
participate. All Inyo County residents are eligible to respond to the survey.

What is a Community Health Needs Assessment?
Nonprofit hospitals conduct a CHNA every three years to better understand the health needs in
their communities. Health data sources and feedback from community members collected
through an anonymous survey process help the hospital to understand diverse community
perspectives, prioritize area needs, and develop an action plan to address those needs.

What is a QR Code and how do I scan it?
A QR Code is a two-dimensional bar code that can be used to directly open a link on a website. To scan a QR Code, open the camera app on your mobile phone. Hold the phone so the QR Code
appears in the camera’s viewfinder. Allow the camera to focus on the code. A notification
window will appear on your screen, asking for your approval to open the link. Tap on the window and wait for the survey to open. Please wait, as it will take a few seconds for the survey to load.
